Support Coordination and Specialist Support Coordination are NDIS funded services that can help build your capacity to understand your NDIS plan and use the funding in the best way to meet your needs. Importantly Support Coordination aims to build your ability to act independently. This can include empowering you to access and co-ordinate your supports and providing assistance to help you participate more in your community. As local people working in the community, Divine Life Care are able to source a diverse range of supports and services that will support you to reach your goals. The NDIA may consider adding Support Coordination to your plan if there are reasons you could experience challenges navigating the NDIS or managing your providers efficiently. Divine Life Care offers two levels of support coordination, providing as much or as little assistance as you need.
Level 2 Support Coordination – Coordination of Supports:
This support will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
Level 3 Specialist Support Coordination:
This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment which may include health, education, or justice services. The Specialist support coordinator aims to reduce barriers to participants implementing or using their NDIS plan as well as ensuring consistent delivery of service.
